I follow the Graphic Artists' Guild's guidelines as outlined in their Handbook of Pricing & Ethical Guidelines.

With most of the artwork done for clients, I retain the copyright (ownership) of the art. Rights are assigned per usage type and extent of usage.

Standard rates are quoted for the intended usage & extent. Unlimited usage rates are typically an additional 100% of the standard rate, and full ownership rates are an additional 200% of the standard rate. Logo work is usually more expensive as the ownership goes to the client.

So for example: a t-shirt design that costs $300 would be $600 for unlimited usage, and $900 for full ownership.
